[ Multiplane Facelift ]
Multiplane Rhytidectomy, Incisional Lifting
A multiplane facelift is an incisional procedure to rejuvenate aging face, pulling up drooping tissue, the main feature of aging, through restoring not only the superficial skin layer but also the deep core soft tissue and SMAS layer. It is definitely different from the previous simple skin lifting or non-incisional thread lift. It can rejuvenate the old tissue into its original young structure through full layered reconstruction for dramatic and long-lasting effects.
JJ plastic surgery has decades of experience in facelift and other rejuvenation procedures. We already published academic article on “Plastic and Reconstructive Surgery” Journal of the American Society of Plastic surgeons, in 1999. Multiplane facelift for Orientals. We also adapted 3D scanner for analysis of facial change with aging. Technically, well experienced faculties with subspecialty can take care of patients.
It takes about 3and 1/2hours under shallow level general anesthesia monitored by board certified anesthesiologist, followed by dressing removal after 3~4 days, and suture removal on postop 7 days. You can get back to daily life within 7 days.

    The 3D facial scanner in JJ is an optical stripe projection method (LED) which is harmless to the human body. It can scan the asymmetry and drooping of the pre-operative face and predict the post-operative appearance in 3D photo.
    Unlike existing 2D-based diagnostic programs, 3D images are used for visualization and counseling. You can relieve the anxiety about the results of surgery and accurately share the goals of surgery between doctors and patients.

Good Candidates for multiplane facelift
  1. Bigger face than the previous young face
    When we were young, the facial soft tissue attachment to the bone was so firm that total volume of the face was minimal. With aging, these close relationship has been loosened progressively. The tissue sagging on both sides of the jaw makes facial contour rectangular, losing previous ovoid V-shaped jawline. This is not a change of the facial bone but the soft tissue problem. These serial changes of the soft tissue at the jaw evoke the stepping deformity and wider facial width. The multiplane facelift could reconstruct the relationship of tissue and bone and make the face smaller again.
  2. Facial foreign body removal
    Most of the facial tissue is attached to the cheekbones. With aging, the cheek depression is the first sign of tissue drooping. So, fillers or fat injections for the compensation are frequently done without concern. However, inappropriately transplanted fat or injected filler could accelerate tissue sagging due to its own weight. The foreign substances or transplanted fat in the cheek tissue could be removed only by the multiplane facelift incision.
  3. Skin redundancy after facial bone surgery
    Facial bone surgery like mandibular reduction, cheek bone reduction, and two jaw orthognathic surgery are frequently done to reduce the facial volume in young age. But, the excessive bone reduction can cause early aging face and soft tissue sagging which hides the surgical change. A multiplane facelift can reduce sagging excessive tissue to correct the inappropriate ratio of the bone and tissue after the facial bone surgery.

    Also, multiplane facelift and facial bone surgery can be done together for facial soft tissue and skeletal reduction at the same time. So even in old age facial bone surgery can be done successfully without cheek sagging.
  4. Neck, jawline, and chin drooping
    The chin and neck are attached to the mobile mandible. The lower part of the chin has many fat layers, so it could sag earlier than other tissue because there is no part directly fixed to the bone. In addition, when the platysmal fascia connected to the SMAS layer falls down to form thick vertical wrinkles, it can be effectively improved with neck incision lifting. (In the case of horizontal wrinkles, you can expect only about 60~70% of the effect.) Furthermore, simple drooping of the fat layers and soft tissues without sagging platysma fascia can be improved simply by neck liposuction and Elasticum lifting.
  5. Nasolabial fold
    The nasolabial fold is the borderline between cheek (mainly soft fat) and upper lip(mainly muscle), so even young person makes the temporary fold when makes smile. Therefore, normal folding of young people does not require any correction.
    With aging progress, the cheek fat drooping over the nasolabial crease deepen the crease into the fold. Even a thread lift is sometimes effective because the region is relatively near the temple although it can last only a few months. Therefore the multiplane facelift can reconstruct the inner soft tissue structure and get a reliable result. However, even by the multiplane facelift, the nasolabial fold improvement is relatively less prominent than another part because it is very active moving part and located at the corner between nose and eyes. So, fat or filler injection is recommended for a compensation.
  6. Jowl Effacement and Jawline straightening
    Jowl is a fold of fatty tissue hanging from the jaw. It is located between two retaining ligaments. Thread lift could efface it for a while, but the effect is not reliable and temporary because the jawline is constantly moving and located below the upper retaining ligament. The jowl drooping below the retaining ligament could be corrected successfully only when the upper ligaments are freed with complete dissection. The Jawline straightening and Jowl effacement can be well achieved by the Multiplane facelift.
  7. Submentum and Neck
    The mandible (lower jaw) has to move frequently to speak, masticate, swallow etc. So, tissue near the mandible is very soft and loosely attached to the bone. The submentum and neck are attached to the undersurface of the mandible and tongue which is no bony frame inside. This part of the face droops earlier than the other. Early submental sagging can be corrected by simple liposuction with or without necklace elasticum procedure.
    However, when the neck muscle (platysma), a continuation of the SMAS falls down to form a thick vertical band (Turkey neck deformity), platysmal repair at the midline is recommended through a submental incision (2~4cm)

  8. Saggy Chin
    Chin is mainly muscular soft tissue firmly attached to the center of the mandible. Although the active muscular structure is relatively resistant to the gravity, tissue in front of the chin is easy to slide down. Furthermore, the previous injection of filler or fatty tissue accelerates the progress. Usually, these sagging can be corrected by the multiplane facelift only, but in a severe case, chin lifting through submental incision procedure is needed to correct the severe Saggy chin.

Pitfalls of “simple” thread lift

Simple the thread lift might be disappointing because.

  1. Limited Effect
    The effect of thread lifting is relatively good at the upper part of the face, but very disappointing at the lower portion, around mouth, jawline, and jowl. The facial soft tissue attached to the skeleton is divided into multiple compartments by several retaining ligament (firmly attached area).

    Usually, thread lift pulls soft tissue drooping upward to the temple. So, the lower face cannot be lifted well enough because of its long distance from the temple and retaining ligament act as a barrier of force.
  2. Limited Longevity
    Since human soft tissue is not sold, holding the soft tissue by a thread is not stable. Furthermore, the tissue movement of mastication, speaking, and facial expression continuously weakens the tissue and thread holdings.
    Since it does not make a reliable adhesion, the tissue holding force easily weakens through massages and daily life.
  3. Foreign body sensation

    The thread has hard and sharp spur for even distribution of tissue load.

    These spurs might evoke pain, foreign body sensation and sometimes protrusion.
  4. Superficial irregularity and dimples
    The tissue near the thread is pulled more strongly, on the other hands, the tissue far from the thread pulled less, so tissue might be distorted producing unwanted superficial iregularity and dimples
Fundamentally, relaxed skeleton and soft tissue connection should be reconstructed by dissection and tissue regenerations, the lifting effect without dissection like thread lift can last only limited period even with unwanted complications.
Multiplane Facelift vs Simple Facelift
In its beginning period, Classic facelift only with a skin excision is frequently done. Due to only a superficial skin modification, the effect could not sustained enough. Furthermore, the scars were prominent by excessive skin tension.

(Lifting the skin only technique cannot overcome the weight of the SMAS and deep tissues, so the effect is limited and scar might be prominent due to skin sion overloading.)
Therefore, underlying soft tissue including superficial muscular aponeurotic system (SMAS) should be redraped into the original young shape to provide a natural and long-lasting effect.
The delicate facial expression muscles, fascia, and fat layers just beneath the skin is called the SMAS layer. SMAS consists of simple tendinous portions, fatty tissues, and the facial expression muscles those make the facial dynamics linked together. It also contributes most of the thick volume of the facial soft tissue. Aging is a phenomenon in which the center of weight moves downward in the form of teardrop from the young original inverted triangular structure. (The wrinkles are rather secondary problem, and the sagging is the most important feature) The incisional lifting has evolved to improve the deep tissues sagging from simple skin pulling. The retaining ligament should be freed to improve lower face
The soft tissue is fixed firmly to the facial skeleton and the masticatory muscle at some specific sites. There are collagen-dense ligamentous structures between soft tissue and underlying bony skeleton. These ligaments are located regularly and fix the facial tissue firmly at those points than others. We call it “retaining ligaments”.
Since the soft tissue saggings are divided by the retaining ligaments, complete and successful lifting needs relocation of ligaments with the incisional approach. The lower part of the face below the upper ligaments cannot definitely lifted with simple thread lifting or simple skin lifting procedure especially areas near chin and mouth

To improve the front of the face above the jawline, the nasolabial line and the anterior sagging, the mini-facelift is used with the only preauricular incision. To improve neck below the jaw, the hidden-facelift only with the postauricular incision is enough. Depending on where you need to lift, the incision might vary, but unnoticeable

JJ multiplane facelift utilizes a special incision known as a transfollicular or trichophytic incision. This incision is made just behind the hairline and done in such a manner that the hair will regrow through the incision scar. The hair follicles are preserved underneath the newly lifted skin flap with meticulous repair. Afterwards, hair shafts will grow through the skin flap over the scar and hide the incision line.
Facial nerves
Because of the risk of facial nerve injury, some surgeons recommend thread lifting, or simple lifting rather than real facelift. However, the facial nerve is located underneath the SMAS layer, there is no risk of the nerve damage if only operated appropriately. In JJ, for 19 years we don’t have any facial nerve injury case.

[ Neck lift ]
People usually show the first signs of aging in their necks, which often leads them to consider a lower facelift or a neck lift. Double chin, turkey neck deformity, vertical banding and horizontal creases all make us look older than we expect. A neck lift enhances the appearance of the neck by tightening the skin and underlying muscles and improving the contour of the jawline. This procedure is often performed in conjunction with liposuction, a facelift, or a cheek lift but may also be done as a stand-alone procedure In order to reach the most aesthetically pleasing results, your neck lift procedure will be thoroughly tailored to your unique contours.












  1. Simple submental Aculaser liposuction for “double chin”
    excess fat around the neck makes the neck and jaw boundary unclear (double chin). Unless the skin and muscle of the neck is loose too much, simple submental liposuction might be a good choice. Procedures are performed under a local anesthesia. Recovery period is within 3days.
  2. Submental liposuction with necklace Elasticum lift for severe double chin and sagging
    Mild skin loosening can be corrected with Necklace Elasticum lift with laser liposuction.
    (Elasticum, Silicone and polyester thread, unlike normal thread, it has elasticity , last longer than conventional one.) Procedures are performed under a local anesthesia. Recovery period is within 3days.
  3. Submental incision with platysmaplasty for Simple Turkey gobbler deformity
    Only with submental incision, the medial edges of both platysma muscle is repaired to obliterate the turkey gobbler deformity (Vertical neck band). The adjacent fat tissue can be removed at the same time. Procedures are performed under a local anesthesia or low level general anesthesia. Recovery period is within 5days.

  4. Full cervicoplasty (submental platysmaplasty+ Postauricular hidden lift)
    The procedure usually begins with liposuction in the neck and jowl regions. Then, operating through small hidden incisions behind the ear, the surgeon lifts and tightens the platysmal neck muscle to rejuvenate the neck contour. Any excess skin is removed from behind the ear. Turkey gobbler deformity (vertical neck band) is usually corrected through the submental incision.

    Procedures are performed under a low level general anesthesia. Recovery period is within 7days. If you do not have excessive weight gain and skin changes due to disease, the effect last for 5 to 7 years.
[ mouth corner lifting ]
Aging cheek drooping includes downward migration of the mouth corners at the edge. Mouth corner lifting is one of the most beneficial procedure to enhance a rejuvenation with a midface lift and facelift procedure for a healthy dynamic tension on the face.

Mouth corner lifing for aging oral angle is the same procedure for a young patient. The depressor anguli oris insertion is partially divided and fixed to those of the levator muscles for an enhancement of elevation and reduction of depression of the modiolus (oral angle). Since the incisions are located just out of the vermilion border (about 7mm), the scars become less noticeable soon.
